Oncoming headlights start looking like they have rings around them. Streetlights bloom into starbursts. Reading a menu in a dim restaurant takes longer than it used to. These changes tend to creep in slowly, which is why many patients don’t connect them to cataracts until an eye exam confirms it.
Halos and glare are two of the most common complaints among patients in their 60s and 70s, and they’re also two of the symptoms that worry people most, especially when night driving starts to feel less safe. The question that usually follows a cataract diagnosis is whether cataract surgery completely resolves these symptoms.

Why Do Cataracts Cause Halos and Glare?

A healthy eye lens is clear, letting light pass through and focus on the retina. When a cataract forms, the lens becomes cloudy and uneven. Light entering the eye then scatters in different directions before reaching the retina.
This scattering effect is exactly what produces halos and glare. A porch light or headlight becomes a diffused source of light rather than a defined point, and the eye interprets the scatter as a ring or burst surrounding the light. Because pupils dilate more in low light, allowing more scattered light to reach the retina, these symptoms are almost always worse after dark than during the day.
What Happens to These Symptoms After Cataract Surgery?
In cataract surgery, the cloudy lens is removed and replaced with a clear artificial lens, called an intraocular lens (IOL). Since the cloudy lens causes the scattering, removing it usually solves the problem. With the new IOL, light focuses properly again, and halos and glare from the cataract usually go away.
Many people notice improvement just days after surgery. Night driving feels safer, headlights look normal again, and reading in dim light gets easier. The timing can be different for everyone, but once the cloudy lens is gone, the main cause of the problem is fixed.
How Your IOL Choice Can Affect Night Vision

Not all intraocular lenses give the same night vision results, so it helps to talk openly with your eye doctor before surgery. Standard monofocal lenses usually give the clearest night vision and the fewest side effects, because they focus light to a single point, as a healthy lens did before cataracts.
Premium lens options are more complex. Multifocal and trifocal lenses split light between different focal points, letting you see clearly at several distances without glasses. This light-splitting can cause mild halos around bright lights at night, especially at first. If you are considering a trifocal or EDOF lens, know that EDOF designs usually give a smoother range of focus and tend to reduce halos compared to older multifocal lenses.
This does not mean premium lenses are a bad choice. You should consider how often you drive at night, how you currently use reading glasses, and how important visual freedom is to you. Talking honestly with your eye doctor about these lens options before surgery helps set realistic expectations for your night vision afterward.
When Halos or Glare Might Linger for a While
Some people, especially those who choose a multifocal or trifocal lens, notice mild halos or starbursts around lights in the first few weeks after surgery. This is normal as your eyes and brain adjust to the new lens, and it usually fades over time. Persistent or worsening glare weeks or months after surgery is a different situation and worth mentioning at a follow-up visit. Can Halos and Glare Be a Sign of Something Else?

Cataracts often cause halos and glare, but they are not the only reason. Dry eye disease can make the tear film unstable, which can scatter light and create halos around headlights and streetlights, even if your lens is clear. Sometimes, people being evaluated for cataracts find that dry eye disease is causing their symptoms, either alongside or instead of an early cataract.
Astigmatism and some corneal irregularities can also cause halos, especially at night when your pupils get bigger. This is why getting an eye exam is more important than trying to diagnose yourself. Two people can have the same symptom but require different treatments because their causes differ.
